This is an exciting month for us, as our first contracts through Anthem and Medicare Shared Savings take effect! We will be working our way together through the journey of new health care delivery models as well as health care payment. The staff of St. Vincent’s Health Partners is committed to assisting the physician and hospital membership with the necessary support to make this a successful transition. The process will evolve over the next two to three years, as we refine the care coordination, and PCP offices transition to Patient Centered Medical Homes. As we go through the journey together, the message to keep in mind is that this will lead to improved patient care, better patient satisfaction, and overall lowering of costs of healthcare. The goal is lofty, but achievable as we work together as a team for improved patient outcomes. I look forward to being a part of this transformation, and working with each of you as St. Vincent’s Health Partners’ physician members take the lead in moving healthcare in Connecticut forward!

As was noted in the CEO’s Corner, both the Anthem and the Medicare Shared Savings contracts have been signed and submitted to Anthem and Accountable Care Associates, respectively. The Anthem contract began August 1, 2013 and the Medicare Shared Savings contract began July 1, 2013. Thank you to all that helped with the signing of the contracts. It was quite a big task and we appreciate the attention that was taken to get them returned to SVHP by our deadlines!
Signed copies of the Anthem contract will be sent to the offices upon receipt from Anthem. Signed copies of the Medicare Shared Savings contracts and paperwork have been forwarded to each office. We will be scheduling a meeting for all PCPs to review the metrics for the Anthem contract over the next few weeks.
The Request for Proposal (RFP) was submitted for SmartHealth. We expect to have an answer from St. Vincent’s Medical Center on their decision within the month.
Contract discussions and negotiations are in process with Cigna, Connecticare and United Healthcare. We will continue to update you through the website, emails and the newsletter.
One of the many benefits of membership in the PHO is the ability to negotiate contracts with third party Payors. If anyone approaches you or your office staff regarding contracts, please contact Karen O’Driscoll, Director of Professional Relations.
Clinical Quality and Care Coordination
The Care Coordination Department of SVHP has been identifying and documenting the best practice processes and communication patterns for transition care in our network. Attention is being directed to the more than 100 transitions which exist for the patients between the multiple care settings. A SVHP ‘Playbook’ with the transitions to be used both as the standard and to reference as a resource, will be available to all members in the near future. The SVHP vision based on feedback from our members embraces the “Triple Aim” goals of quality cost-effective care with patient satisfaction and reflects the Institute of Medicine’s description of high quality care:
¡ Timely (without unnecessary delays)
¡ Safe (transitions are planned, managed at point of care)
¡ Effective (based on best practices)
¡ Patient-centered (respectful of patient preferences)
¡ Efficient (the right care at the right time in the right place and avoiding unnecessary duplication)
¡ Equitable (the standards apply to all patients)
Education for the SVHP Playbook will be available as part of the Monthly Education Program being held on October 3, 2013 from 6:00-8:00 PM.

Practice Administrator/Office Manager Meeting
On Friday, August 16th from 9:00-10:30 AM, SVHP will host its first Practice Administrator/Office Manager Meeting. The meeting will take place in the Board Room of St. Vincent’s Health Partners, 2754 Main Street, Bridgeport. Items on the agenda include St. Vincent’s Health Partners website, Anthem and Medicare Shared Savings Contracts and Availity. Remember to use the Physician Directory Search area when recommending physicians to your patients. The directory can be searched by Physician name, City, Zip Code, Specialty and/or Practice Name.

SVHP Staff Update
Please help me welcome Jacqueline Jacovino to the SVHP staff as our new Administrative Assistant. Jacqueline has provided administrative and legal support to upper level management for over 25 years. Jacqueline is a graduate of the Connecticut Business Institute and also attended Three Rivers College for two years focusing on the Business Administration program. She brings a diversified skill set ranging from office management to graphic design and works closely with our staff to ensure our day-to day office operations. Former employers include Pullman, Comley, MPTN Tribal Nation and Sandy Shore Motel & Apts. Most recently she designed and produced local marketing campaigns as a freelance artist for a variety of clients. Jacqueline has strong ties to her community and serves as volunteer to Shakespeare Theater and Bread of Heaven ministry.
