Contract Updates

Both SmartHealth and the Medicare Shared Savings contracts are effective January 1, 2014.

Discussions continue with HealthyCT, Aetna, Connecticare and United Healthcare. We will keep you upraised of updates through the website, emails and the newsletter.

One of the many benefits of membership in the PHO is the ability to negotiate contracts with third party Payors. If anyone approaches you or your office staff regarding contracts, please contact Karen O’Driscoll, Director of Professional Relations.

Provider Engagement and Transitions of Care Educational Series

Health reform is happening at a rapid pace. To aid the membership in keeping up with the latest regulations and information on health reform, St. Vincent’s Health Partners, in partnership with Fairfield County Medical Association, offers educational programs with CMEs available.

The following is a list of programs that are scheduled for 2014:

¡     “Your Role Within a Clinically Integrated Network”—This program is being presented by a consultant with AHRQ and will discuss the role and responsibilities of each provider within a Patient Centered Medical Neighborhood model.  The discussion will address not only the roles of the PCP but also the Specialists, the Skilled Nursing Centers, the Home Care Agencies and Pharmacies and how to work together to put the patient at the center of care and achieve quality outcomes.

¡     “The Health Care Delivery System:  A Look at Alternative Methods” – This program will address alternative delivery systems such as telemedicine, telehealth, minute clinics and the impact it will have.

¡     “Motivational Interviewing”

¡     “ICD-10”

¡     “Cyber Liability and Billing Audits”

Practice Manager Quarterly Meeting

St. Vincent’s Health Partners Quarterly Practice Managers meeting was held on Friday, December 13th.  Dr. Raskauskas presented information on ICD-9 coding and the importance of coding correctly. Coding cheat sheets for Diabetes and CHF were handed out to each practice that attended. The SVHP Playbook, Transitions of Care and expectations were also discussed.

SVHP Practice Highlight

SVHP is excited to announce that Dr. Hervey AWeitzman has joined the PHO!

Dr. Hervey A Weitzman, MD, has served the greater Bridgeport Community for over 35 years. Dr. Weitzman has experience in medical practice specializing in Oncology, Hematology, Pathology and Internal Medicine.

Dr. Weitzman’s undergraduate years were spent at the University of Massachusetts Amherst. He attended medical school at the University of Vermont. Initially, he trained and worked in both anatomic and clinical pathology. His fellowship training has included such institutions as the Albert Einstein School of Medicine, the Hospital of the University of Pennsylvania, Brown University and Memorial Sloan-Kettering Hospital.

At this time, Dr. Weitzman practices in Internal Medicine only, but his extensive background empowers him to diagnose and treat his patients with excellent care and guide them to sound health. His office is located at 4699 Main Street, Bridgeport, CT.  Phone:  (203) 374-0277.

Access Health CT Enrollment Fairs

Everyone needs health care coverage. Help enrolling is available. SVHP’s Assister, Christina Cano, RN, BSN, IPA, will be enrolling individuals at Enrollment Fairs to be held at St. Vincent’s Family Health Center, 762 Lindley Street on the following dates:

Saturday, January 11th, 9:00 AM-5:00 PM

Saturday, January 25th, 9:00 AM-5:00 PM

On Tuesday, January 7th, from 9:00 AM -1:00 PM, there will be an Enrollment Fair at FSW, Inc. CT, 475 Clinton Ave, Bridgeport.

Staff Updates

Please help us welcome Gretchen Cavaliere as the newest member of the SVHP staff! Gretchen is the Administrative Assistant to the CEO/President of St. Vincent’s Health Partner. Prior to her employment with St. Vincent’s Health Partners, Gretchen worked for twelve years with Casey Family Services as an Executive Assistant and Accounts Payable. She has over seventeen years’ experience working in the Administrative field. She is also currently attending St. Vincent’s College School of Nursing to pursue her bachelor’s degree.
