Happy Holidays2

The Staff of St. Vincent’s Health Partners wishes all our members Safe and Happy Holidays!!

As we reflect on the past year, we have achieved many successes and milestones within the PHO! The year started off with growing our number of employees from 2 in January to 8 employees as of December.

Excitement continued with the Beta Site visit from URAC and the signing of the McKesson’s Population Manager and Risk Manager IT platform. As of this writing, the User Acceptance Training (UAT) for Population Manager is in the final stages, with last test scripts being tested.

April brought a new “home” for the staff as we moved from the basement of St. Vincent’s Medical Center to our new office at 2754 Main Street. A Ribbon Cutting and Blessing Ceremony was held and we are filling the space quickly with Care Coordinators, Assisters and IT Analysts.  We also hosted our first Provider Education Program on April 18th, which was a success.

In May, we signed our first quality based contract with Anthem. This was also the first quality based contract of its kind in the state and for Anthem.

June, July and August passed by at a quick pace with the addition of the Medicare Shared Savings contract, 2 educational seminars, the start of the practice managers quarterly meetings and the beginning of speaking engagements for the SVHP staff!

The rollout of the Population Management Reports happened in September and we have already seen results moving in the right direction for the management of our patients!

In October, we hosted our first Annual Board Retreat at the Bridgeport Holiday Inn and the Board agreed on a strategic plan for 2014.  We also enrolled our first participant in the Connecticut Health Insurance Exchange.

The month of November brought the end of contract negotiations with SmartHealth, and successful signing of a quality based contract with SmartHealth. The end of November began the beginning of our URAC Clinical Integration Accreditation, as we “hit” the submission button for our application.

As the year comes to a close, we want to thank our Board of Directors for their leadership and our members for our collaboration partnering together to achieve the same goal:  cost-effective, quality care to our patients focused to achieve the best possible outcomes!

We look forward to a Successful 2014 and reaching the many goals that we have set!

Contract Updates

Discussions are moving forward with HealthyCT and we expect to have further information about a contract by the end of the month.

A proposal has been sent to United Healthcare and we are awaiting notification from them.

Discussions continue with Aetna and Connecticare.  We will continue to update you through the website, emails and the newsletter.

One of the many benefits of membership in the PHO is the ability to negotiate contracts with third party Payors. If anyone approaches you or your office staff regarding contracts, please contact Karen O’Driscoll, Director of Professional Relations.

Practice Manager Quarterly Meeting

St. Vincent’s Health Partners will hold a quarterly Practice Managers’ Meeting on Friday, December 13th from 9:00 AM-10:30 AM, at 2754 Main Street. Access Health CT Enrollment Fairs

Everyone needs health care coverage. Help enrolling is available. SVHP’s Assister, Christina Cano, RN, BSN, IPA, will be enrolling individuals at Enrollment Fairs to be held at St. Vincent’s Family Health Center, 762 Lindley Street on the following dates:

SVHP Practice Highlight

St. Vincent’s Health Partners is excited to announce that Dr. Rachel Sheiman has opened a new pediatric office conveniently located off of I95 at 325 Reef Road in Fairfield, called Penfield Pediatrics!

Penfield Pediatrics strives to provide high quality, comprehensive care for our patients and families in a warm, personal environment. Patients will benefit from a small setting with a Board Certified Pediatrician who can really get to know their family and provide continuity. Penfield Pediatrics offers a more intimate model of care. Dr. Sheiman focuses on taking time with her patients and their needs.

Office hours are: Monday through Friday 9 AM-5 PM. Saturday and Sunday for urgent visits by appointment only.

Terry Hillman is the RN Coordinator and has worked with Dr. Sheiman for over 7 years.  The office number is (203) 696-3580; fax:  (203) 696-3584.

All health insurances are accepted, including Husky.

A graduate from Trumbull High School and longtime resident of Fairfield, Dr. Sheiman is excited to be home and immersed in the community in which she resides! As a mother of two young children, she integrates her experience as a parent in the understanding of her patients, parents and their feelings. She recognizes how stressful parenting can be and treats all her patients like her own.  Having been in pediatrics for 17 years, she incorporates her vast knowledge of the field with her compassion for others.

Dr. Sheiman graduated from Trumbull High School in 1988 and went on to receive her BA from Cornell University in Ithaca, NY in 1992.  At that time, she ventured to Chicago to get her medical degree from Loyola School of Medicine in 1996. She completed her residency at the Connecticut Children’s Medical Center (“CCMC”/UCONN) in Hartford in 1999.

After residency, Dr. Sheiman worked at New England Pediatrics, which has offices in Stamford and New Canaan, until June of 2006 at which time she joined Canterbury Pediatrics in Monroe. While at Canterbury Pediatrics, she worked as a pediatric consultant at Southwest Community Health Center in Bridgeport from 2008 until 2013. She remained at Canterbury Pediatrics until June of 2013, and then joined St. Vincent’s Multispecialty Group. Penfield Pediatrics opened in August 2013. She is excited to bring her 14 years of practice experience to Fairfield!

Dr. Sheiman is Board Certified by the National Board of Medical Examiners and the American Board of Pediatrics. She is a Fellow of the American Academy of Pediatrics and a member of both the Fairfield County Medical Association and the Connecticut State Medical Society.

Hospital Affiliations include St. Vincent’s Medical Center, Bridgeport Hospital and Yale New Haven Hospital.

As a member of SVHP, Dr. Sheiman appreciates the concept of Patient Centered Medical Home and engaging the patient and their family in their care. Her commitment to care is evidenced by her quality outcomes.  Please consider referring your patients to Dr. Sheiman/Penfield Pediatrics.

SVHP Staff Update

Please help us congratulate Colleen Swedberg, MSN, RN, CNL in her new role as Director of Care Coordination and Integration. Colleen has accepted this position effective December 9, 2013. Colleen has been an integral part of the SVHP team in guiding the practices with Care Coordination and Population Management.

Please also help us wish Brenda Reig well, as she vacates the Director of Clinical Quality position.
