ACO & Payer Leader Summit

Colleen Swedberg, VP, Strategy at SVHP was invited to be a presenter at the ACO & Payer Leader Summit September 17, 18 in Irving, Texas. She spoke on the experience of culture change through the experience of the SHVP network in a presentation entitled:  The Influence of Person and Perception – Culture, Data and Clinically Appropriate Decision Making 

To surpass meaningful quality and cost goals of care, patients have to be able to access care at the right place and right time. This has never been more apparent to healthcare businesses in the USA than now, as payment models move towards value based design. Providers in all settings typically struggle to provide the information patients need to make educated decisions about where they will get their care. While data can shed light on problematic patterns, culture can be just as critical to sustainable change, as identifying the problems. The session explored how the organization through the experience of BPCI specifically, has been successfully closing the cultural gap between hospital, post-acute and ambulatory services, which had been preventing patients from accessing evidence based care in the right setting.

  • Culture is king – harnessing the impact of expectation
  • Data-driven strategy and behavior change – identifying clinically appropriate issues and decisions to impact real change in quality, cost
  • Collaboration and communication – identifying meaningful metrics, educating partners and capitalizing on technology to enhance day to day decision making

The Summit’s program topics were validated by senior executives from the Health Plan and ACO executive community as the top critical issues they face. Key issues for the 2018 included:
