Health Equity Workshop

SVHP staff Jessy DiDio, RN Care Coordination & Integration Associate, and Ricardo DeSouza, Accountable Health Communities Program Screener,  attended the Health Equity Workshop: Developing Effective Partnerships to Address Social Determinants of Health onsite at the Connecticut Hospital Association in Wallingford, CT this past Friday, April 27th, 2018. Hospitals are forging collaborative relationships with key partners in their communities to improve care and achieve health equity.  Partnerships between hospitals and key community-based organizations are central to addressing social determinants of health, which are tied to effective healthcare and improved patient outcomes.  In this workshop, Collaborative Consulting, a specialist consultancy with particular expertise in partnership development between healthcare and community organizations,  provided information on models and strategies for effective hospital/community-based organization partnerships.

Jessy and Ricardo, commenting on the event, remarked that the information that was presented validated the experience of the SVHP network and the development of a model that incorporates evidence based strategies which address social determinants and the achievement of health equity.
