Contracting

We are in the final stages of negotiation with the Anthem and SmartHealth contracts.  It is anticipated that both contracts will become effective July 1, 2013.

St. Vincent’s Health Partners is also in discussions with Cigna, Connecticare and Healthy CT.  These contracts, like the Anthem contract, will be quality based and will have Care Coordination components to them.

The goal of the PHO is to have approximately seven contracts by the end of 2013.

Clinically Integrated Platform

The MedVentive project kickoff meeting was held on 3/21/13, and included SVHP and SVMC. The project is now in the discovery phase and will include SVMC, several physician practices, lab, pharmacy, radiology information gathering and an onsite visit from the MedVentive project team at a TBD date. The discovery phase is scheduled to be completed in 8 weeks and will lead to the system design phase.

The goal is to have 50% of the membership up by July 1st, 75% of the membership up by September 30th and 90% of the membership up by December 1st.

Over the next few weeks we will begin to reach out to your practices to schedule training for MedVentive.

 

Website

The St. Vincent’s Health Partners website (www.stvincentshealthpartners.org) is up and running.  The website has two views, a public view and a private view.  All providers will be receiving an email within the next month with details of your username and a temporary password.  Once you receive your information, you will be prompted to change your password.  The passwords will have certain criteria, which will be explained in the email.  Once, you have logged in, please view all of your information to ensure that it is correct.  If you need to make changes, there is a “request change” tab that will send an email to us of your updates.

Office Updates

We are happy to announce that SVHP has moved into our new office on 2754 Main St, which is on the corner of Main and Hawley.  Construction was completed in record time, thanks to Kuchma Construction, Ronald Brown, Jr. and John Connolly, we were able to move in on Friday, March 22nd.

Many thanks to SVMC for allowing us to have our offices in various locations throughout the last nine months.  We will miss our daily interactions with many of you but encourage you to visit our new home!

A Ribbon Cutting Ceremony and Office Blessing were held on Thursday, March 28th.  Bill Hoey, VP of Missions and Ethics, gave the Opening Prayer.  Susan Davis, CEO of St. Vincent’s Health Services and George Goldfarb, MD provided Opening Remarks, followed by the Ribbon Cutting and Blessing of the Cross by Father Al Forlano.

An Open House is being planned for the end of April and an invitation will be sent out within the next few weeks.

SVHP Committees

All SVHP members are encouraged to participate in one of our committees.  There are currently 3 working committees for you to consider:

Quality and Utilization Review Committee: The Quality and Utilization Review Committee assists the board in overseeing, ensuring and improving the quality and appropriateness of clinical care, patient safety, and satisfaction provided throughout the organization, and in shaping a strategy to achieve a high level of health throughout the greater Bridgeport community.  The Committee meets the third Wednesday of the month at 7:00-8:00 a.m.

Information Technology Committee: The Information technology committee assists the board in overseeing the development of Health Information Technology Infrastructure and data management plans necessary to maintain high quality care and improve patient health. It meets the fourth Monday of the month at 6:00-7:00 p.m.

Population Health Management and Finance Committee:  The Population and Health Management and Finance Committee assists the Board in creating a population health strategy, managed care contracting, and fiscal and oversight duties. It meets the second Thursday of the Month at 6:00-7:00 p.m.

New Employees

Colleen Swedberg , RN, MSN joins our team as a Care Coordinator. Colleen is a Certified Clinical Nurse Leader who has expertise in care coordination with a particular interest of improvement of quality and financial outcomes by strengthening partnerships with clients that promotes safer health self-management. She is an adjunct professor in public health at Fairfield University and comes to us with many years of Case Management experience.
