Annual Meeting Recap

On Monday, May 11th, St. Vincent’s Health Partners hosted it’s 2nd Annual Meeting and Board of Directors Elections in the Hawley Conference Room of SVMC.  Many of our Provider members were on hand as well as representatives from our Skilled Nursing and Home Health Agency Post Acute Partners.

Dr. Raskauskas acknowledged the Board, thanked Dr. George Goldfarb for hi service as Chairperson over the past 2 years.  John Gleckler, SVP and CFO of SVHS, was introduced as the Incoming Chairperson.

Dr. Stuart Marcus, President and CEO of SVHS spoke to the members about the current climate of healthcare in the Fairfield region.  He stressed the importance of the Clinically Integrated Network working together  especially as we enter into Bundled Payments and Value Based Purchasing.

The Annual Report was presented followed by Bylaw changes.  Dr. Raskauskas then spoke about looking ahead into 2016 and potential challenges in relation to reimbursement, competition, attribution, population health management and care coordination.

The Board of Director open positions and nominees were announced, followed by voting of the members.  Dr. Howard Sadinsky, DO, Pediatrician with The Milford Pediatric Group was elected to the PCP vacant seat for a 3 year term.  Dr. David Yan, MD, Plastic Surgeon with Fairfield Plastic Surgery, LLC was elected to the Specialist vacant seat for a 2 year term.

The meeting was adjouinred after the elections were completed.

Transitions Leadership Committee Update

The Transitions Leadership Committee continues to meet on a monthly basis to discuss opportunities to strengthen relationships to improve transitions of care.  This month’s meeting was held on Thursday, May 21st

The objective and focus of the meeting were reviewed and follows:

  • The Transitions Leadership Committee will discuss opportunities to strengthen relationships to improve transitions of care.

In recognizing the tremendous efforts of this team, we will highlight positive improvements and communications throughout the network each month. Examples from this month’s meeting include the following:

  • HHA seeing a large increase of in-network referrals
  • Telephonic huddles are going very well; members are appreciative of the content and the expedient nature
  • Primary Care Specialists has seen an increase of SVMC and SNFs calling to set up post-D/C appointments with PCP within 7 days; they appreciate this as is save them time chasing down the patient to set appointments up
  • Dr. Weisman of Pulmonary and Internal Medicine instructed his staff that they could not refer out of network unless they first consulted with him

Updates were presented on Transition Issue Tracking, and the Network CQI processes for SNFs and HHAs.

Moving forward, the committee will regularly track metrics as a way of quantifying the work that is being done.  Proposed metrics and communication methods are

  • Readmissions
  • In-network Referrals
  • Length of Stay
  • W10 Tracking
  • Weekly REadmission Eamils
  • Electronic Flagging of Patients

Additional metrics may include

  • Post D/C MD appointment within 7 days of discharge
  • PCP communication with all transitions
  • Effect of use of an APRN pilot program on readmission rate
  • Dollar amount saved in every BPCI case

We anticipate that the work that is done in these meetings will positively overflow to the transitions of all patients between each of the settings and across the clinically integrated network.

The next meeting will be held on Thursday, June 25th from 1:00 PM-2:30 PM in Hawley 1 Conference Room.

Medical Specialists of Fairfield has offices at 425 Post Rd, South Lobby in Fairfield, at SVMC 2800 Main Street, 3rd Floor, SWIM Hematology Oncology Center and Commerce Park, 4699 Main Street, Bridgeport.  The Fairfiled and SWIM offices are open Monday through Friday from 9:00 AM- 5:00 PM.  The Commerce Park office is open Monday, Tuesday and Thursday from 9:00 AM -5:00 PM.
