CEO’s Corner

2014 is looking to be a very busy year for St. Vincent’s Health Partners and our physician membership! As the Affordable Care Act is being implemented through the Health Insurance Exchange, and the Connecticut State Innovation Model Design Grant is being developed, we are moving away from volume based care to value based care and new reimbursement models. In addition, federal legislation requiring electronic prescribing, meaningful use and PQRS reporting with resultant penalties in Medicare reimbursement for non-compliance are adding to the difficulties of private practice. Finally, the implementation of ICD-10 in October this year will strain practices to their limits.

St. Vincent’s Health Partners is here to help practices navigate these changes, provide resources to our membership, and do what we can to ease the administrative burdens faced in the practice of medicine. I feel where there is change, there is opportunity. Together, we can work to address all the requirements needed to accomplish those changes, yet still enjoy the practice of medicine. I look forward to the challenges this year will bring, and we will be working with each of our physician members to individualize the needs of your practices.

Please make sure to put our first annual meeting scheduled for March 6, 2014 from 6-9 p.m. on your calendar! We will be electing Board members, and presenting our first annual report. Please don’t hesitate to contact me directly with any questions or concerns you have related to SVHP, and I welcome your input into how we can assist you better!

Provider Engagement and Transitions of Care Educational Series

Health reform is happening at a rapid pace. To aid the membership in keeping up with the latest regulations and information on health reform, St. Vincent’s Health Partners, in partnership with Fairfield County Medical Association, offers educational programs with CMEs available.

We are excited to have Michael P.  Johnson , PT, PhD from BAYADA Home Health Care and a member of the National Advisory Council and the Agency for Healthcare Research and Quality (AHRQ), as our speaker.

Within a Patient Centered Medical Home, Primary Care Physicians develop an ongoing relationship with the patient.  The PCP directs care between a team of health care professionals to assure the healthcare needs of the patient are being met.  The Patient Centered Medical Neighborhood is the collaboration between specialists to improve care integration and coordination within the PCMH model.

Michael will be discussing the concepts of clinical integration,  population health and the role of the Specialists and Sub Specialists to ensure effective communication, coordination and integration within PCMH practices.

SVHP Practice Highlight

The SVHP Neighborhood continues to GROW!!  Please help me welcome Dr. Aimee Altschul-Latzman, with Allergy and Asthma Consultants of Fairfield County, LLC, located at 140 Sherman Street, 3rd Floor, Fairfield, (203) 955-1461.

Dr. Aimee, Altschul-Latzman practices both Adult and Pediatric Allergy, Asthma and Immunology.  After completing her undergraduate degree at Yale University, Dr. Altschul-Latzman earned her Doctor of Medicine degree at Albert Einstein College of Medicine, where she was elected to AOA, the National Medical Honor Society.  She then spent six years at the Mount Sinai Medical Center in New York, NY, where she completed her residency training in Internal Medicine and Pediatrics, followed by her fellowship training in Allergy and Immunology.  She is board certified by the American Board of Allergy and Immunology and was also board certified by both the American Board of Internal Medicine and the American Board of Pediatrics.

Immediately following her training, she spent five years in private practice in Manhattan. She then moved with her family to Connecticut and spent the past six years in private practice in Fairfield, Connecticut.  Dr. Altschul-Latzman is an attending physician at St. Vincent’s Medical Center in Bridgeport, Connecticut.  She was also an attending physician at the Mount Sinai Medical Center in New York, NY, for five years.

She has a special interest and expertise in food allergies, having completed her fellowship training under world-renowned experts Drs. Hugh Sampson and Scott Sicherer at the Mount Sinai Medical Center in New York City. Dr. Altschul-Latzman authored several original peer-reviewed publications in leading medical journals and educational resources.

Access Health CT Enrollment Fairs

Everyone needs health care coverage.  Help enrolling is available.  SVHP’s Assister, Christina Cano, RN, BSN, IPA, will be enrolling individuals at Enrollment Fairs to be held at St. Vincent’s Family Health Center, 762 Lindley Street on the following dates:

Saturday, February 8th, 10:00 AM-5:00 PM

Saturday, February 22nd, 10:00 AM-5:00 PM

On Tuesday, February 4th,  and Tuesday, February 18th, from 9:00 AM -1:00 PM, there will be Enrollment Fairs at FSW, Inc. CT, 475 Clinton Ave, Bridgeport.

Enrollment Fairs will be held at the Bridgeport Public Library, 925 Broad St, Bridgeport on Thursday, February 13th and Thursday, February 27th from 2:00 PM – 7:00 PM.

Access Health CT Enrollment Success Story

Client A signed up for health insurance through Access Health CT and decided to choose a bronze plan.  The cost of the plan was approximately $600 per month (which was the lowest costing plan).  Client A contacted Christina Cano regarding the plan.  Christina researched the plan to verify it was the best plan for the client.  Upon investigation, it was determined that the Client had not received proper tax credits.  Once the tax credits were applied to the application, the monthly premium for the bronze plan was $0.

As of January 31st, Christina has enrolled over 250 people, and each one is a success story!
